Requirements for 1st time applicants:

  • Duly accomplished Online Application form
  • Formal letter of application
  • Two (2) sponsorship from PHILTOA member Company in good standing
  • Application processing fee: Php 1,000 (Non-refundable)
  • Annual membership dues Php 6,000
  • Authorized letter stating the name of authorized representatives. Representatives must hold managerial position.
  • Biodata of authorized representative with 2×2 pictures
  • Copy of LGU license or Mayor’s permit
  • Copy of SEC Articles of Incorporation & By-laws
  • Lastest GIS or DTI certificate
  • List of company owners, officers and employees with designation
  • Photo of Office/Property (Exterior and Interior)
  • Copy of Office Space Lease of Contract (Applicable for regular members only)
  • Must be operational for at least one (1) year.
